The nurse's office is located in the main office area. Our school nurse, Janice Hoverstad, is usually at St. Mary's School Monday-Friday from 9 AM - 1 PM Outside of these hours, the student should report to the main office.
Nurse Janice is responsible for collecting health records, developing emergency health plans, and meeting with students about health related problems, first aid, or minor health issues. NOTE: The nurse's services are not to take the place of the family doctor.
Please review each section below for important health information and forms.
GUIDELINES FOR KEEPING SICK CHILDREN HOME:
If your child is sick, he/she should stay home. Review Guidelines for Keeping Sick Children Home in determining whether your child should stay home in order to promote quicker recovery and prevent the spread of illnesses at school.
IMMUNIZATIONS:
Each student must be up to date on immunizations. Parents, please provide our school nurse with the most up-to-date Immunization Record for your child. You may bring this to your physician to complete or you also can call Minnesota Immunization Information Connection (MIIC) at 651-201-3980 for an updated record.
HEALTH HISTORY:
The nurse will need an updated Health History Form annually for each student. This is needed to provide the best care for your child.
CHRONIC HEALTH CONDITIONS / EMERGENCY ACTION PLANS:
We are committed to providing safe, individualized care to students with special health considerations. Communication with the school nurse is essential in ensuring that your child's health needs are managed appropriately in the school setting.
The development and implementation of your student's emergency care health plan allows for proper nursing interventions, and for the sharing of necessary information regarding your child's safety while at school. All student health plans are renewed annually at the beginning of each school year and as needed in response to any change in student health status.
If your child has a chronic medical condition (examples listed below) please notify the health office so an Emergency Action Plan can be developed.
MEDICATIONS:
Students may not self-administer medications at school. All medications taken by students during school hours must adhere to appropriate guidelines:
